our mission

To improve the quality, efficiency, and access of healthcare and social services through the use of secure, mobile, real-time exchange of clinical information between healthcare teams and their patients.

secure messaging

Facilitates real-time secure communication with team members, patients and clients.  Encryption at every level of the communication stream.  Professionals, patients, and clients are validated within the platform to ensure accurate transmission of data.

communication boards

Designed for efficient communication within groups or institutions.  Users simply message into specific categories within a single dashboard.  Messages are distributed to users based on their permission to see a specific category.

virtual clinics

Secure video-conference based meetings, used for clinical interactions or for secure interactions with clients or customers.  Originally designed for healthcare team members to meet with their patients, the virtual clinic concept is limited only to the users imagination.

ACT

The Acute Care Transition Clinic™

ACT is the Clinomics ... virtual clinic. It is designed as a support tool for acute care systems that are under strain. When ERs and inpatient facilities are overcapacity, they can become difficult places to be for both patients and healthcare providers. Patients are often discharged into the community without adequate community follow-up, leaving them feeling abandoned and vulnerable. Acute care physicians and healthcare team members often feel that system pressures prevent them from providing optimal care, leading to disillusionment and burnout. The ACT is a virtual support structure for both patients and team members while at the same time building capacity within the system.

watch

™

what is a Clinomics ... watch clinic?

watch combines the advanced work-flows of the ACT clinic with the patented functionality of the category based communication structure of Clinomics ... .   It allows the healthcare team to monitor specific health parameters while patients are at home.  These may be simple clinical parameters such as blood pressure or HbA1C or more complex clinical information such as reporting symptoms after being discharged following abdominal surgery.  It is especially helpful for conditions which require serial observations such as a cellulitis, a healing fracture, or  starting a new medication.  It allows for efficient, cost effective care that improves quality, while respecting the time of our patients.

assist

™

helping physicians access specialist advice through

Clinomics ...

assist

Rural and community based physicians can access Clinomics... specialists simply by messaging or faxing a consult to the Acute Care Transition Clinic (ACT).  This can be simply for advice on a patient that they are managing in the community or to add a patient to the specialist's virtual clinic where they can actively participate in the management of that patient's condition.  For example, surgeons can assess the need for non-emergent surgical conditions and discuss the pros and cons of surgical intervention.  This may be particularly helpful for patients experiencing  geographic or mobility challenges.  Similarly, Internists may choose to manage a patients chronic disease virtually so as to keep a closer eye on them via a "watch" clinic.
